Record the block you actually chose
The sample CSV describes a hypothetical 100-question, 240-minute practice block with a 20-minute review reserve. It records answering time 220 minutes and an average answering budget 2.2 minutes per question. Those are selected practice inputs.
Use a reason that guides a correction
Fields distinguish reading, concept selection, assumptions, calculation, entry and review. A note that only says wrong answer cannot identify whether to study a concept or change an execution routine.
Compare a later attempt on compatible terms
Retain question source, block size, duration and review allocation. A shorter block or a different question mix can change the result without proving a general performance improvement.
Keep the record’s role explicit
The file is a log, not an official mock or a pass predictor. It helps choose a targeted next task. Use confirmed exam-format information separately when planning a full-format rehearsal.
