Record the attempt before rewriting it
Keep the original answer, time context and the step you found difficult. Then inspect the explanation. This separates what you did independently from what became clear after reading the solution.
Match each error with a corrective task

Include uncertain correct answers
A correct guess is different evidence from a correctly explained answer. Mark uncertainty so the topic is not removed from review only because the selected letter happened to match.
Make the follow-up observable
Name the next question or exercise and write what a successful independent attempt would demonstrate. A mock score can describe that attempt; it does not by itself provide a reliable pass probability for a different exam.
