Use the worksheet
Download the CSV and open it in a spreadsheet application. Two completed examples demonstrate the fields. Replace those examples with your own practice records. Keep descriptions specific enough that a later review tells you what to practise, rather than simply recording that an answer was wrong.
Classify the cause
Separate conceptual misunderstandings from calculation conventions, reading errors and time-management issues. A wrong rate interval needs different practice from a missing definition. Record the correction in your own words, then select a focused task that can show whether the correction worked.

Check the correction on a new task
Reattempting an identical question can reward memory rather than understanding. Choose a related task with a meaningful change in what you must do, then explain the solution. Update the log when you can complete the task reliably. The worksheet is a review aid, not a readiness score.
